REQUIREMENT

Certification form

A separate supporting letter/memorandum, for each nominee, from the President or her/his designee

No more than five letters of recommendation, at least one from the department chair or dean

A current and relevant c.v.

A one-page essay from the nominee on her/his contributions in the nominated category

Documentation of exemplary achievement (such as student evaluations, student testimony, peer critiques, research citations, reviews)

Additional supporting material NOT TO EXCEED 15 PAGES

Any other material requested by the Institutional Faculty Nominating Committee, the President, or the Regents' Faculty Awards Committee

Numbering of all pages in the portfolio

Submission of SIX three-hole punched copies of each nomination portfolio

Materials clipped together or held together with rubber bands PLEASE DO NOT STAPLE OR BIND MATERIALS, AND DO NOT SUBMIT MATERIALS IN NOTEBOOKS OR BINDERS.
