NOMINATION-PORTFOLIO CHECKLIST
All nomination portfolios must meet the following standards. Thank you in advance for your cooperation.

A separate supporting letter/memorandum, for each nominee, from the President or her/his designee |
|
|
No more than five letters of recommendation, at least one from the department chair or dean |
|
|
A current and relevant c.v. |
|
|
A one-page essay from the nominee on her/his contributions in the nominated category |
|
|
Documentation of exemplary achievement (such as student evaluations, student testimony, peer critiques, research citations, reviews) |
|
|
Additional supporting material NOT TO EXCEED 20 PAGES |
|
|
Any other material requested by the Institutional Faculty Nominating Committee, the President, or the Regents' Faculty Awards Committee |

|
Numbering of all pages in the portfolio |
