To act on your allegation, we need you to provide as much identifying information
as possible via your comments and documentation.
The more you can tell us, the better chance we have of investigating and
resolving the allegation.
Who Can Submit a Report Through the Fraud Hotline?
Employees, vendors, contractors and any other interested party.
When Should an Incident be Reported?
Referrals should be made to the hotline when you first learn of the fraud, waste, or
abuse. Keep in mind, the longer the time between the incident and the time it
is reported, the more difficult it becomes to investigate.
